FUNDING FACT SHEET FOR CAREGIVERS

 

The following funding options are a general guide of county and state programs that may pay for our adult day care services. *Please note that each program has specific criteria.

 

#1 DEPARTMENT OF HEALTH FUNDING

This funding is granted on the specific diagnosis of Alzheimer’s disease, multi-infarct dementia, or Parkinson’s disease. A single client, living with family cannot have an income, which exceeds $15,162. yearly. For a married couple living with family the couple’s income cannot exceed $19,827. yearly. For a married couple living alone joint income cannot exceed $24,493. yearly. Senior Care will complete the application for funding when we obtain verification of social security, pension, interest bearing statements and income tax returns (if one was filed for prior year). For a husband and wife, we need to verify the above for both individuals. An original, not a faxed copy, of the client’s diagnosis must be in place before we can start the application process. The Department of Health will fund up to three (3) days each week, with a $14.75 co-pay per day. There is a wait list for this funding.

 

 

#2 ALZHEIMER’S ASSOCIATION RESPITE GRANT

One time grant of $500. specifically for a diagnosis of a memory impairing disease.

There is no financial piece to this application. To apply please contact: Mr. Fred Brand at the Greater NJ Chapter of the Alzheimer’s Association (973) 586-4300. Their address is: 400 Morris Avenue, Suite 251, Denville, NJ 07834. There is a wait list for this grant.

 

#3 STATE-WIDE RESPITE CARE PROGRAM

This program is designed to give relief to caregivers of the frail elderly and/or functionally impaired persons who are 18 years or older. The program in Essex County is administered by the Essex County Division on Aging. To apply, please contact: Ms. Loretta Melito at (973) 395-8384. The respite application includes a medical certification and a financial profile which requires supporting documentation; a copy of social security check, pension check, or copies of direct deposit statements. This program has a cost share (co-pay) based on the client’s income. Financial criteria is as follows: the client’s income cannot exceed $1,536 monthly, and the client cannot have more than $40,000. in liquid assets. Respite care will pay for two (2) days of social adult day care, or one (1) day of medical adult day care. Senior Care is a social adult day care program. There is a wait list for this funding. Income guidelines change every year.

 

 

#4 COMMUNITY CARE EXPANSION PROGRAM FOR THE ELDERLY AND DISABLED (CCPED)

This is a community-based Medicaid program. Eligibility requirements are as follows: the client must be age 65 or older, blind, or determined eligible for disability by Social Security. Monthly income cannot exceed $1,536. Applications can be obtained from Ms. Butler (973) 395-8183. Once completed applications are returned to: Ms. Butler at the Essex County Division of Welfare/Suburban Services, 50 South Clinton Street, 2nd Floor, East Orange, NJ 07018. Services include adult day care, case management, home health care, respite, and non-emergency medical transportation. This program offers a combination of services that our caregivers find very helpful. There is a wait list for this program.

 

#5 JERSEY ASSISTANCE FOR COMMUNITY CAREGIVING (JACC)

This is a new program which provides in and out of home services (case mgmt, homemaker, respite care, home modification, special medical equipment & supplies, chore service, personal emergency response system, home delivered meal service, caregiver/recipient training, adult social day care, adult medical day care, attendant care, home-based supportive care, & transportation) to seniors at risk of placement in a nursing facility. JACC includes an array of services designed to supplement assistance given by the individual’s caregiver. JACC participants work together with a case manager to develop a Plan of Care to best meet their needs. The case manager works with the caregiver and participant to ensure that services are delivered as planned. Eligibility criteria includes: care recipient must be a NJ resident age 60 and older, require nursing facility level of care but wishes to remain at home, is not financially eligible for Medicaid or Medicaid Waiver Services, has resources at or below $26,000 for an individual, or $52,000 for a couple, is a US Citizen or Qualified Alien, is willing and able to share in the cost of services (participants in JACC share in the cost of their care, on a sliding scale based on income), and finally has no alternate means to obtain the needed support.
